For all pre-existing medical conditions requiring Cover-More's approval before cover can be granted you must answer questions 1-31. If you wish to apply for cover for a known heart condition your doctor needs to answer questions 33-52 as well. All questions must be answered or the application may not be able to be assessed.

If you require help with the completion of this form, please consult your doctor for medical information or Travelinsure for other details.

If you are over 18 years of age, this form must be signed by you, if someone is signing on your behalf, we will require a copy of the legal authorisation that indicates that they may do this for you (eg Power of Attorney documents).

If your doctor completes any question on this form for you, they must sign the doctors declaration. You must still sign your medical authority and declaration at question 30.

Any costs associated with submission of this application are your responsibility.

Once you have completed this form please return it to Travelinsure, Shop 5, The Strand Village Centre, Mt Eliza VIC 3930 or FAX it to us at (03) 97871145. Within a few days of receiving application a letter will be sent advising of Cover-More's decision or one of our nurses will telephone you.

POLICIES DO NOT AUTOMATICALLY PROVIDE COVER FOR TRAVELLERS WITH AN EXISTING HEALTH DISORDER

An Existing Health Disorder means any medical or dental condition of yours or any person on whose state of health your trip depends, which in the 30 days before you applied for this insurance:

• required treatment, investigation, medication or advice from a doctor, chiropractor, physiotherapist, naturopath, psychiatrist or
• which you are aware of, or could be expected to be aware of, that may lead to a claim under this policy

It also means a chronic or on-going medical or dental condition. Existing health disorder cover is only available in respect of medical conditions for the persons insured under this policy.

Assessment of Application

Please download an assessment application form above or contact Travelinsure on 1300 654 956 to have one sent to you.

Complete the Form and return it to Travelinsure, Shop 5, The Strand Village Centre, Mt Eliza VIC 3930 or by fax on (03) 9787 1145 for assessment and processing. The medical section will need to be completed by your usual doctor.

Only the conditions which follow are automatically covered under this policy, provided there are no planned procedures, and there have been no complications or exacerbations of the condition within the last six months. In any other circumstance, an Existing Health Disorder application form is required.

Acceptance / Authority

Compusure Pty Ltd will fairly assess applications but cannot guarantee acceptance. If cover can be provided we will advise the terms of cover and plan code, together with an Acceptance/ Authority Number. Cover of Existing Health Disorder is limited to the approval for the original policy and cannot be extended. If accepted, an additional premium of $60 on International Plans and $40 on Domestic and New Zealand and Domestic Cancellation and Emergency Expenses Plans will apply per approval. Acceptance/Authority Number must appear on the certificate of insurance.
